The Manager of Case Management will operate with integrity, initiative, positivity and a motivation for self-direction with understanding of direct report and leadership communication needs. The right candidate must possess the ability to exercise supervisory skills and leadership creativity while maintaining a positive work environment to create and maintain an engaged, competent, effective, and efficient Case Management team.Knowledge of the Medicare/Medicaid and Joint Commission regulations and standards as related to acute care Case Management and ability to execute and maintain a team with effective and efficient practices to meet regulations and standards. Experience in Transition of Care preferred. Establish and maintain effective, collaborative, positive, and consistent working relationship with post-acute providers in transition of care activities.Essential duties include: Assess, update, manage, report, and maintain a culture of evaluation and improvement that meets CMS Conditions of Participation (and Joint Commission) expectations for discharge planning, transition of care, and utilization management. Maintain patient rights related to Case Management including, but not limited to: Discharge Planning; Medicare discharge appeal right; post-acute provider patient choice.
Education: Bachelors degree in Nursing (BSN) or Masters degree in Social Work (MSW) required. Experience: Five (5) years experience in the area of specialty, and two (2) years of supervisory/management experience. Skills: Acting with integrity, analyzing and interpreting financial data, computer literacy, gathering needed information, delegating and supporting teams, evaluating decisions/solutions, improving work process, making effective decisions. Excellent human relations and oral/written communications skills. Measuring customer service and meeting customer needs. Partnering with customers/others, promoting NMC mission, vision and values. Utilizing communication channels and Valuing diversity. Licenses/Certifications:Required: Current AZ RN Licensure or compact state RN license or LMSW license.Preferred: Case Management or Utilization Management certification.
