The Clinical Discharge Planner acts as Clinical Case Manager/ Discharge Planner for the System Hospitals. To facilitate timely and cost efficient discharge planning. Oversee team of Discharge Coordinators to ensure they have proper support and resources to assist with discharge planning needs to the patients.
Collaborates with social workers, medical team, payers, referral sources, home health agencies, DME/HME companies, and intake staff to facilitate patient discharge and coordinator of care. Serves as hospital liaison for discharge coordination and maintains good working relationships with all entities.
Supervises Discharge Coordinators to assist with coordination of services for patients to facilitate timely and accurate discharge planning.
Provides patient/caregiver education for discharge planning requirements and options. Provides continuing education programs as needed.
Provides ongoing support and supervision to the discharge coordinators to ensure they have the proper training and resources to perform their responsibilities.
Communicates clinical follow-up information to the medial team as appropriate. Including any disruption in service, post discharge issues and resolutions. Participates in the institutions quality assurance/performance improvement initiatives as requested.
Respond to requests for clinical information, to assist in the precertification and authorization for discharge needs.
Demonstrated computer input expertise, including the ability to navigate and find information in the Meditech system and other computer systems needed.
3-5 years in a clinical settings, supervisory experience
Preferred Home Health experience.
Degree in Nursing, RN preferred
Licensed Registered Nurse, in the State of Illinois
Licensed Clinical Professional Counselor, utilization review for psychiatric unit
Must be licensed to practice in chosen discipline by the State of Illinois or eligible for licensure as stated by state guidelines and meet all state licensure requirements
