You will be required to be in the field going into facilities or homes up to 40% of the time to meet with members. Your territory will be within a commutable distance of your home, 50 miles or 50 minutes. When not in the field, this will be a telecommute / work from home position. Candidates must reside in the state of Delaware. Primary Responsibilities: Making outbound calls to assess members' current health status Identifying gaps or barriers in treatment plans Providing patient education to assist with self-management Interacting with Medical Directors on challenging case Coordinating care for members Making referrals to outside sources Coordinating services as needed (home health, DME, etc) Educating members on disease processes Encouraging members to make healthy lifestyle changes Documenting and tracking findings Field visits with members
Required Qualifications: Current, unrestricted RN license in the State of Delaware Field based experience to include home health, hospice, community based care, correctional facilities, etc. 3+ years' experience in a hospital setting, acute care, direct care experience as an RN and/or experience as a telephonic Case Manager for an insurance company Computer proficiency utilizing MS Office, including the ability to type and talk at the same time Experience working with both Pediatrics and Adults Reside in the state of Delaware Ability to be in the field and travel up to 50 miles or 50 minutes from your home.Preferred Qualifications: Bachelor's Degree Experience working with special needs or vulnerable population Experience working in Psychiatric care Behavioral Health Experience Experience working in Substance Abuse Background in managed care Case Management experience Certified Case Manager (CCM) Medicaid ExperienceCareers with Optum.
