When not in the field, this will be a telecommute / work from home position. You will be in the field approximately 40% of the time meeting with members and working from home 60% of the time. Primary Responsibilities: Making outbound calls to assess members' current health status Identifying gaps or barriers in treatment plans Providing patient education to assist with self-management Interacting with Medical Directors on challenging case Coordinating care for members Making referrals to outside sources Coordinating services as needed (home health, DME, etc) Educating members on disease processes Encouraging members to make healthy lifestyle changes Documenting and tracking findings Occasional field visits to members This position will require active and unrestricted Nursing licensure in multiple US States. Required Qualifications: Current, unrestricted RN license in the State of Delaware Multiple state licensure (in addition to Compact License if applicable) or ability to obtain multiple state nursing licenses required 3+ years experience in a hospital setting, acute care, direct care experience as an RN and/or experience as a telephonic Case Manager for an insurance company Computer proficiency utilizing MS Office (Word, Excel, PowerPoint and Outlook), including the ability to type and talk at the same time while navigating a Windows environment Strong Pediatrics and Adult clinical background Reside in the greater New Castle County, DE areaPreferred Qualifications: Bachelor's Degree Experience working in Psychiatric care Behavioral Health Experience Experience working in Substance Abuse Background in managed care Case Management experience Certified Case Manager (CCM) Medicaid Experience Experience working with special needs populationCareers with Optum.
