Provides direct service to elderly clients needing assessment and Case Management to remain safely and comfortably in the community. Completes home visits in the community, hospital(s) or in HUD 202 buildings. Proficiently conducts assessments on lap top in clients' homes; provides information; develops realistic Care Plans; authorizes governmental services; refers and links clients; and monitors client services with supervisory guidance. Follows the standards and guidelines of the Illinois Department of Aging, the Area Agency of Aging, Housing and Urban Development (HUD) and the Council on Accreditation. Skillfully utilizes computer systems; types service logs and case notes; and enters data electronically with supervisory prodding. Prepares billing information in an accurate and timely manner.
Preferred Requirements: BA/BS 3 years of experience. Specific Certifications.
Minimum Requirements: BA/BS/BSW. Specific certifications.
Other Requirements: Advanced level of keyboarding and word processing.
