General Summary/basic PURPOSE OF JOB: The Case Manager functions as a core member of a collaborative care team in a developing system of care (SOC). The Case Manager will diligently do outreach to locate identified clients and assist them in accessing various needed services. The Case Manager will ensure the coordination of patient and family care with both internal and external, partner healthcare organizations. Coordination will include but may not be limited to, behavioral health, psychiatry, primary care, substance/alcohol abuse, school-based services, in-home services, faith-based providers, Medicaid Managed Care Organizations and social determinants of health for such things as housing, legal and food insecurities. The position will be community based and be required to travel within the communities to meet with our clients and to coordinate healthcare. The Case Manager should be highly organized and be able to quickly establish relationships with patients, families and healthcare providers.
MINIMUM Education: · bachelor’s degree in behavioral health (e.g., psychology or social Work) · 2-years/care management and coordination experience · Excellent networking and communication skills · Excellent record keeping skills · driver’s licensE and good driving record · reliable vehicle · comfortable working in the communities.
MINIMUM WORK EXPERIENCE: · 2-years of case/care management in behavioral health field
KNOWLEDGE & SKILLS:
Master’s prepared behavioral health providers will be considered
Knowledge of health care provider organizations on the southwest side of Chicago (SOC defined area)
Past experience in the coordination of care between a wide variety of provider types (e.g., primary care, schools, faith-based organizations, behavioral health, etc).
