The Clinical Case Manager will support the interdisciplinary team in facilitating patient care, with the underlying objective of enhancing the quality of clinical outcomes and patient satisfaction.--Graduation from an Accredited School of Nursing. --Current RN license. --BSN for external candidates and per diem conversions. --Current work experience with discharge planning, utilization management, case management, performance improvement, and managed care reimbursement. --Experience performing in-depth analysis. --Experience interacting with clients/customers in person and/or by telephone. --Extensive experience working in healthcare environment. --Computer skills performing data collection and data entry functions. --Professional skills providing a positive customer service oriented experience. --Skills in reading and comprehending English text and medical terminology. --Writing skills. --Mathematical skills performing calculations. --Knowledge of medical and nursing treatment plans, levels of care, etc. --Knowledge of Medicare, MediCal, and HMOs, MIA, County, and third party payor contracts. --Knowledge of Title XXII, The Joint Commission, and Lumetra criteria.
