Primary Responsibilities:Manage long-term care Medicaid population in nursing facilitiesMaintain contact and schedule appointments to nursing facilities and engage with membersWork within all Florida State guidelines with regard to care and case managementNotifies Long Term Care Facility designated staff, providing the skill details both telephonically and electronicallyPerform other duties as assigned
Required Qualifications:High School Diploma / GED (or higher)4+ years of LPN experience in geriatric, long term care or case managementCurrent and unrestricted LPN for the State of FloridaAbility to use a Windows PC and learn new applications easilyAbility to create, edit, and send documents using Microsoft Word, Microsoft Excel, and Microsoft OutlookAccess to reliable transportation and the ability to travel to nursing facilities Must live in Flagler, Volusia, Saint Johns, Duval or Clay countiesAbility to travel up to 75% of the timeThe Optum Complex Care Management team delivers care for high risk, medically complex populations. Our programs include ISNP, IESNP, TTS, LTC and some D-SNP. The goals of our programs are to improve the overall quality of care, decrease the need for transitions between levels of care (especially avoidable hospitalizations), and to support the member in developing a plan of care consistent with their overall goals of care. The care team for the ISNP program is led by the Advanced Practice Clinician (APC). APC partners with the member's primary care physician (PCP), the member and / or family / authorized representative, and nursing home staff to provide primary / preventive care and personalized care coordination while proactively identifying triggers for acute illness and managing chronic conditions.
