Provide daily leadership/clinical consultation and assistance to case managers.
Responsibilities and Duties
Provide training on case management services to new employees and assign case management referrals. Lead the treatment team in implementation of ITP goals and review/sign completed ITPs and MHAs. Develop goal oriented ITPs that are strength-based and recovery oriented on all assigned clients as required. Assist in developing services and manage a caseload of clients; evaluate consumers for appropriate services and make all necessary referrals, both inside and outside the agency. Provide accurate, quality documentation within a maximum of 48 hours after service delivery. Actively participate in clinical supervision.
Master’s degree in human service field with LCSW or LCPC license and minimum two years clinical experience required. Must have a valid Illinois driver's license, auto insurance and automobile.
Hours: Monday – Friday, 9:00 – 5:00, 35 hour work week. Including benefits.
