To link patients to specialty care, provide support for the healthcare providers, office staff and practice manager. The patient navigator works under the supervision of the Care Management Supervisor. This position coordinates medical needs of patients from physician to specialist offices as well as schedules medically necessary procedures for patients. This position also covers, if needed, the medical receptionist positions.
High school diploma or GED equivalent
3 years experience in outpatient clinic, healthcare environment or other clerical position.
Experience with PC software applications in word processing, Epic, Word, Excel
