This is the place to do your life's best work.(sm) As a Cancer Support Program (CSP) RN Case Manager in our Lisle, IL office, you will review patient status over the phone while ensuring they are receiving the best oncology care. This role is for an 8 hour shift (40 hour work week) and the hours for this role will be Monday - Friday, within the hours of 8:30am-7pm with occasional consumer-centric hours that may be later. Primary Responsibilities: Making outbound calls to assess oncology patients' current health statusIdentifying gaps or barriers in treatment plans Providing patient education to assist with self-management Interacting with Medical Directors on challenging casesCoordinating care for members Educating members on disease processesEncouraging members to make healthy lifestyle changesDocumenting and tracking findingsMaking post-discharge calls to ensure that discharged member receive the necessary services and resourcesUtilization management through evaluating and approving the medical necessity, appropriateness and efficiency of requested health care services, procedures and cancer treatments under the provisions of the applicable health benefits plan
Required Qualifications: A current, unrestricted RN license in the State of Illinois 3+ years experience in a hospital setting, acute care, direct care experience OR experience as a telephonic Oncology Case Manager for another insurance company 1+ years of clinical experience dealing with oncology patients Chemotherapy administration experience Preferred Qualifications:BSN OCN certification A background in managed care Case or disease management experience Experience / exposure with discharge planning Certified Case Manager (CCM) Bilingual (Spanish/English) skillsComputer proficiency, to include strong data entry skills and the ability to navigate a Windows environment Careers with Optum.
