Primary Responsibilities: Visit members in their homes and / or other settings, including community centers, hospitals or providers' offices Provide a complete continuum of quality care through close communication with members via in-person or on-phone interaction Support members with condition education, medication reviews and connections to resources such as Home Health Aides or Meals on Wheels Assist members with the transition from a care facility back to their homeMust be willing to travel (Up to 80%) within Miami Dade and Monroe Counties (Little Havana, Miami Beach, Hialeah, Florida Keys).
Required Qualifications: Current, unrestricted RN license 3+ years clinical experience in a hospital, acute care, home health / hospice, direct care or case management Computer / typing proficiency to enter / retrieve data in electronic clinical records; experience with email, internet research, use of online calendars and other software applications Bilingual English / Spanish Access to reliable transportationPreferred Qualifications: Case management experience including Certification in Case Management Home care / field based case management Experience working with the needs of vulnerable populations who have chronic or complex bio-psychosocial needs Bilingual skills Experience in Home & Community based or Long Term Care services deliveryIf the hired individual resides in Florida (office based or telecommuting) this position requires the AHCA Level II background check (fingerprinting) by the State of Florida for all clinicians that have direct face to face contact with members OR employees who will have access to confidential patient data and will require renewal every five years.
