The Chicago Family Case Management Coordinator is responsible for the following activities/goals; such as to help participants or their caregivers learn to accept responsibility for their own lifestyle and promote their own health. To enhance the participant’s of their caregivers’ strengths and resources by teaching them skills for seeking out and using individuals and agencies in the community who are available to meet a wide variety of human needs. As time passes the participants or their caregivers will ideally participate more actively, while the Case Manager Coordinator adopts a more supportive role.
Provide nursing intervention and follow-up care for clients identified as High-Risk, in coordination with physicians and other health care professionals
Develop and maintain ongoing communication with available community resources;
Respond to inquires concerning agency policy, rules, and regulations for the CFCM program.
Able to maintain monthly records and reports and submits them as requested
Able to seek and form links with Community Organizations that are able to provide Human Services for the program participants
Able to coordinate and supervise Case Managers, and Outreach worker
Able to develop on-going training of Case Managers, Outreach Worker and ancillary staff according to the MCH service code
Conduct quality assurance activities and monitor the following areas and identify improvement needs; daily clinic operations, CFCM reports, Nutrition education (secondary education methods and materials used), breastfeeding promotion and support and MCH integration plan.
Plan, operate, execute and organize the CFCM program in accordance with State policy
Conduct quality assurance activities, complete reports, as needed and make recommendations for improved services
Serve as a local agency liaison to Cornerstone support hotline
Participate in State and Regional CFCM conference calls and meetings
Communicate to staff any CFCM program updates and reinforcement of program requirements
Determine staff training needs and coordinate education opportunities
Conduct outreach activities to prospective CFCM participants
Establish standards for breastfeeding promotion and support as endorsed by USDA
Ensure that staff can provide women with breastfeeding education, counseling, promotion and support information and activities during prenatal and postpartum periods.
Responsible for the overall administration of the programs to assure compliance with all State policies and federal regulations.
Supervise, evaluate and direct CFCM staff by ensuring that staff at all levels are competent to complete job specific duties when providing CFCM program services
Attend professional conferences, seminars, workshops to update staff on current MCH nutrition practices and other information relevant to CFCM
Identify and collaborate with the local providers and other community partners
Manage assigned caseload per IDHS Grant agreement
Other related duties as assigned
Bachelors of Science in Nursing Degree
Professional Registered Nurse in the State of Illinois
2-5 years directly related experience as a Nurse Case Manager. Supervisory experience preferred
