Primary Responsibilities: Make outbound calls to assess members' current health status Provide a complete continuum of quality care through close communication with members via telephonic interaction Identify gaps or barriers in treatment plansPartner with members and their providers to close gaps and prevent avoidable admissions / readmissionsProvide patient education to assist with self - management Facilitate optimal health and well-being thru condition education, medication reconciliation, and connection to appropriate resourcesThis is high volume, customer service environment. You will need to be efficient, productive and thorough dealing with our members over the phone. Strong computer and software navigation skills are critical. You should also be strongly patient-focused and adaptable to changes.
Required Qualifications:Active, unrestricted RN license in the state of North Carolina 3+ years of clinical experience as a Registered Nurse in any setting Bilingual in English and SpanishIntermediate / proficient level of experience with MS Office applications Must be willing / able to obtain CCM within 2 years of employment Must be able to commute to the Miami, FL office on a daily basisIn order to work from home after training period, must have designated, distraction-free area in home for home officeMust have access to high speed internet (DSL or Cable) Preferred Qualifications:BSN Experience in managed care, case management and / or disease management Certified Case Manager (CCM) Knowledge of disease processes for chronic illnesses in the senior population Home Health experience Knowledge of Medicare guidelines and regulationsPassion and commitment to excellence Positive attitude and solution-oriented Ability to handle multiple tasks and function in a high volume environment Ability to work well as part of an interdisciplinary team If the hired individual resides in Florida (office based or telecommuting) this position requires the AHCA Level II background check (fingerprinting) by the State of Florida for all clinicians that have direct face to face contact with members OR employees who will have access to confidential patient data and will require renewal every five years.
