The individual has overall responsibility for coordinating the daily activities of the case management team, while overseeing the clinical plan of care to conform to evidence-based practice and/or regulatory/payer requirements. This position integrates the case management functions (assessments, care coordination, utilization management, and discharge planning). Works effectively with healthcare team members, administrative personnel and medical staff; Responsible to Case Management Director; serves as primary resource in the absence of Case Management Director. Performs miscellaneous duties as assigned.
