Position Summary: Incumbent is responsible for high level assessment, planning and transitions of care for patients with acute and/or chronic psychiatric conditions.
Rate Minimum $95,985.36 Maximum $114,714.72
Required Qualifications: Ability to establish and maintain effective working relationships across the HealthSystemAbility to facilitate and lead interdisciplinary roundsAbility to maintain a work pace appropriate to the workloadAll external candidates must have a Bachelor of Science in NursingCurrent CA RN licenseDemonstrate experience developing and executing treatment plans for patientsDemonstrated experience performing discharge planning in a hospital or long term medical care facilityExcellent written and verbal communication skills in EnglishExperience with and understanding of Utilization Review criteria and discharge planningFamiliarity of Joint Commission, CMS, CDPH and CCS requirementsKnowledge of and ability to work within a complex health care systemMinimum five years of clinical / direct patient care experience OR minimum 1 year experience working as a RN Case ManagerMust demonstrate customer service skills appropriate to the jobMust possess complete understanding of acuity levels for specific patient populationMust possess the skill, knowledge and ability essential to the successful performance of assigned duties
