Phoenixville Hospital is a 151-bed facility that provides comprehensive medical services through approximately 25,000 emergency room visits, 8,200 inpatient admissions and more than 500 community outreach programs annually. In addition, our Regional Rehab Center ranked in the Top 10 percent of 781 inpatient rehabilitation facilities.

Clinical Case Manger for Phoenixville Hospital Shift: Full-time, 8 hour day (40 hours per week) - The Clinical Case Manager, under the direction of the Director of Case Management, serves as an interdisciplinary team member to coordinate the delivery of health care services leading to quality outcomes for the inpatient hospital population and their families with the limits of the financial and social resources available to the patient and his family. - Coordinates the overall interdisciplinary plan of care for a patient, from admission to discharge. - Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively. - Relies on experience and judgment to plan and accomplish goals. Performs a variety of tasks. - Typically reports to a head of a department/unit.
Education: ADN required; BSN preferred Experience/Knowledge/Skills: Minimum 3 years of acute care clinical experience. Minimum of 2 years Clinical Case Management/Utilization Review experience (preferred) Theoretical knowledge of the nursing process, case management and continuity of care Ability to maintain adequate physical and mental health to meet job performance standards Advanced assessment and teaching skills. Motivation for self-direction Ability to exercise supervisory skills and demonstrate leadership Able to relate and work with chronically ill and disabled patients of all ages, and with emotionally upset, and at times hostile people within the facility. Able to relate and work with various managed care representatives, defining the continuing need for medical treatment. Licenses/Certificates: Current RN license by the Pennsylvania state board of nursing Certified Case Manager (CCM) preferred
