University Health Services (UHS), a dynamic, data-driven, patient-centric college health practice serving Notre Dame students is accepting applications for a FULL-TIMECLINICALCASEMANAGER (10-month position). This is an exciting time at UHS! We've recently concluded a reorganization to enhance student services and improve staff support—and our Clinical Case Management program is a result of that process. This cutting edge program is instrumental in helping us realize our vision of “exceptional college health care, infused with our Notre Dame values, to foster lifelong well-being” through the coordination of care of Notre Dame students with complicated medical conditions, and through student outreach. Reporting to the UHS Director, Clinical Case Managers identify students with complex chronic and/or acute medical conditions, and coordinate care needed to assist students to maintain independence. They partner closely with UHS providers to effect optimal patient outcomes, and make referrals to appropriate levels of care to on- and off-campus providers, ensuring a warm hand-off of UHS patients. As confidential health care providers, the Clinical Case Managers help students navigate their academic responsibilities through collaboration with, and referral to, our partners in the Health & Wellness Unit. Clinical Case Managers also coordinate UHS' wellness screening program, curate an extensive resources list, and maintain clinical proficiency in the UHS clinic. In addition, the Clinical Case Management program has an exciting, new outreach component and will offer student programming (i.e., travel preparation, injury and illness prevention, wellness promotion, etc.), provide hall staff training, advise health-related student groups, and support UHS' marketing initiatives. Our Clinical Case Managers will play a lead role in UHS' large-scale student outreach efforts, such as the Flu Vaccine Blitz, pre-participation physicals for student-athletes, and mass TB testing for incoming students. The successful candidate will be an enthusiastic patient-advocate who enjoys working with a young, healthy population, and who is passionate about facilitating the best possible health care for our students. He or she will possess exceptional interpersonal and written communication skills necessary to interact with multiple levels of stakeholders, including students, providers, campus partners, parents, etc. This newest member of the UHS family will demonstrate the ability to simultaneously handle and successfully manage multiple tasks and initiatives. With the addition of the Clinical Case Management program, UHS is evolving toward a patient-centered medical home model of care. Our comprehensive services include primary care, sports medicine, psychiatry, travel medicine, women's health, case management, immunizations, allergy desensitization, etc. We are supported by in-house Walgreens pharmacy, diagnostic laboratory, x-ray and physical therapy. Students can access services through both scheduled appointments and urgent care triage. To meet our students' needs when the department is closed, we offer an after-hours nurse advice line, transportation and even an apartment for patient recuperation! Our greatest asset is our team of 30 passionate health care professionals, including physicians, psychiatrist, nurse practitioner, clinical case manager, registered nurses, patient care assistants, radiology techs, front office staff and leadership team, all working together to provide the best college health care possible to our students. UHS is housed in beautiful Saint Liam Hall, a fully AAAHC accredited, state-of-the-art college health center located just steps away from Notre Dame's iconic Golden Dome.
Minimum Qualifications: Current or eligible Registered Nurse licensure in the state of Indiana and graduate of accredited school of Nursing. Experience in an outpatient or hospital setting. Ability to work effectively as a member of a team dedicated to outstanding care of patients. Ability to educate a diverse student population on areas of self-care, wellness, follow-up treatment, etc. Ability to handle sensitive information with a high level of confidentiality and discretion.
Preferred Qualifications: Formal case management experience is not required, but is highly preferred, as are a bachelor’s degree or higher, familiarity with the principles of health promotion, and college health experience. (#LI-POST)
