Community Transitions RN Case Manager - Boston, MA

Join us and help people live healthier lives while doing your life's best work.(sm) As a Community Transitions Program Case Manager, you will have the best of both worlds. You have the flexibility to work from home as well as provide member care in a direct care setting. This role will require visits to Hospital, multiple times per week. If you are located in the Boston, MA area, you will have the flexibility to work from home* as you take on some tough challenges. Primary Responsibilities:Visit members in the hospital to engage in post hospital programMake concerted effort to find members post hospital that are difficult to reachMake initial post hospital call within 3 days of discharge to ensure that discharged member receive the necessary services and resources Identify gaps or barriers in treatment plans Provide patient education to assist with self-management Interact with Medical Directors or member's PCP on challenging cases Coordinate care and services for members Make referrals to outside sources Educate members on disease processes Encourage members to make healthy lifestyle changes Document and tracking findingsPrioritize large volume of member activities Adapt to regular process changes as new program evolves

Required Qualifications:Current, unrestricted RN license in state of residence5+ years of experience as an RN in a clinical settingBasic level of experience with Microsoft Word, with the ability to navigate a Windows environmentMust have a designated quiet workspace in your home (separated from non-workspace areas) and is able to be secured to maintain Protected Health Information (PHI) and/or Protected Information (PI)Must have access to install secure high speed internet (minimum speed 5 download mps and 1 upload mps) via cable/DSL in home (wireless/cell phone provider, satellite, microwave, etc. does NOT meet this requirement)Access to reliable transportation and ability to travel to visit members within Boston, MA. *Financial assistance for relocation will not be provided Preferred Qualifications:Bachelor's DegreeA background in community care or home careCase Management experience Certified Case Manager (CCM) Experience with discharge planning or transitional care services Strong organizational skills and multitasking abilities will be keys to success Careers with UnitedHealthcare.
