This position entails managing a caseload of homeless families participating in the Rapid Rehousing Program. The participating families have multiple barriers requiring intensive support services to assist them in securing and maintaining housing, becoming financially stable, and keeping their families intact.
Primary Duties and Responsibilities:
Manage a caseload of 12-15 economically disadvantaged families requiring supportive services through the Rapid Rehousing Program under a Housing First approach.
Provide initial client intake and assessment to include documentation of financial and program eligibility for clients.
Conduct formal inspections of client residences, based on HUD Housing Quality Standards.
Maintain monthly contact with landlords to assure client compliance w/ lease requirements.
Maintain a housing stability plan with clients and update on a monthly basis.
Develop a network of social services, link clients to appropriate resources, and provide advocacy and support.
Instruct clients in basic life skills including home maintenance, comparative shopping, budgeting, laundry, health/hygiene, nutrition, parenting, self-esteem, communication, and other topics as needed/requested.
Monitor and support client progress through scheduled home and office visits, telephone contacts, and post-exit follow-up.
Document case management services through detailed and thorough case notes and updates to housing stability plans.
Complete reports as required by funding sources, monitoring agencies, and the
Participate in required staff meetings and monthly recertification meetings.
Other related duties as required.
Provide excellent supportive services to clients that will result in meeting individual client goal plans of self-sufficiency.
Develop and utilize a broad network of resources to best service clients.
Display a professional role model for clients.
Maintain accurate and up to date files on all clients.
This position requires a minimum of a Bachelor’s Degree in Human Services, Social Work, Psychology or a related field from an accredited college or university. A Master’s Degree is preferable. Requires a minimum combination of education and experience that fulfills the knowledge and experience base required for this position.
