ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.
Case Management and Supervision
Coordinates the patient's and families care and services including those of the team, social worker, chaplain, home health aide, volunteer and physician.
Monitors the re-certification / re-authorization process.
Participates in Interdisciplinary Group Meetings as indicated.
Ensures and documents appropriate use of the medications, durable medical equipment and supplies.
Educates patients, families and other caregivers.
Provides skilled nursing as determined by the interdisciplinary plan of care.
Appropriately utilizes the skills and resources of contract personnel such as PT, OT, Speech and Pharmacy.
Identifies appropriate care plans and integrates care plans as needed
Conducts supervisory visits of Hospice Aides
Participates in the on-call rotation and weekends as needed
Participates in QAP
Participates in PIP as requested.
Participates in the orientation of new team members as assigned by the Clinical Manager.
Attends regularly scheduled in-services and educational conferences.
Develops and achieves professional growth goals and objectives and reviews with the Clinical Manager.
Maintain a safe and productive working environment following established protocols
Promotes and supports a teamwork culture and environment internally and externally
Ensure interactions remain positive and professional with all persons with whom you interact in the course of your work both internally and externally

EDUCATION and/or EXPERIENCE:
Registered nurse with current licensure to practice professional nursing in the state.
Registered Nurses shall have a minimum of (a) one  year of experience as a professional nurse within the last three  years; or have a baccalaureate degree in nursing from a program accredited by the National League for Nursing and a current RN license.
Maintains a current CPR certification
