Post Acute Complex Care Manager – Case Management – Altamonte

The Post-Acute/Complex Manager works under the direction of the Director of Post-Acute/Complex Care to plan, organize, implement and evaluate the complex discharge arrangement process at Florida Hospital. Arranges complex post-acute services for hospital patients in accordance with CMS and ACHA guidelines, in cooperation with the director, Care Management Department, and all other operational internal and external providers. Supervises the discharge arrangements staff in implementing complex discharge plans established by the Care Management team. Oversees the team to ensure the next level of care for the patient is finalized and communicated to the Care Manager. Adheres to departmental and organizational goals, objective and standards of performance, and policies and procedures, continually ensuring quality patient care and regulatory compliance. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.
Proficient in time management with superior prioritization skills
Self-motivator and quick thinker who is proactive and results oriented. Responsiveness to ever-changing matrix of hospital needs and acts accordingly.
Ability to use discretion when discussing personnel/patient related issues that are confidential in nature
Strong competence in Excel, PowerPoint, and Word

Critical thinking and problem-solving skills.
Working knowledge with Cerner, NaviHealth, EOS, Suncare, QlikView, FirstLook, CNO Dashboard
Bachelor's degree in nursing
Minimum 1-year supervisory experience
Minimum 3-years functional Care Management or related area
Licensure, Certification, or Registration Required:
Florida or Multi-State Department of Health Nursing License
ACM- Accredited Care Manager (Preferred)

Provide operational oversight for transitioning complex patients to post-acute settings. Responsible for seamless process deployment and relationship maintenance with Care Management and Post-Acute providers
Creates synergies, maintains multi-directional communication, and acts as a liaison between stakeholders for placement of patients with complex barriers to discharge, including payor source, social, clinical and limited access barriers
Manages and leads front line clinical complex team
Manages contractual agreements with post-acute providers to ensure cost effective and appropriate placement
Works in collaboration with Integrated Health Services to ensure appropriate utilization of PPN and healthcare resources, including escalation process for non-compliance
Responsible for hiring staff, performance management, coaching, discipline, engagement and talent management
