The Lifestyle Center is an on-site Occupational Health setting utilizing a multidisciplinary approach through Medical, Rehab and Fitness programs providing Occupational Health and Wellness to employees and contractors to reduce injuries and illnesses and promote wellness through a holistic approach. The Nurse Case Manager will be responsible for assessing, planning, implementing and coordinating all case activities associated with an injured individual to evaluate the medical and disability needs. Acts as a liaison with injured person, employer, provider(s), TPA, and health care personnel.
Plans a proactive course of action for all work and non-work-related injuries
Coordinates, monitors, evaluates and documents assessment, treatment planning, including medical and rehab services, and return to work processes with the employee, providers, and employer
Performs pre-certification and authorization process for prescribed treatment by gathering relevant data and information through injured employee, provider and employer.
Identifies barriers to recovery and potential alternatives to care and communicates to appropriate parties
Aids Health Care team regarding investigations and the resolution of complaints
Acts as liaison between employee, employer, provider(s), and TPA
Prepares and maintains incident/ injury report, written case records, reports, and forms, performs case follow-up and closing, and performs other administrative tasks as required
Dependent on work load, may be asked to perform other duties as related to department
Adheres to all appropriate privacy, security, and confidentiality policies and procedures
Adheres to licensure and professional scope of practice
Travel may be required, less than 20% of time
Performs other duties as assigned
Demonstrated experience in Case Management
Demonstrated experience in working with multiple groups
Certificate from an accredited Nursing School or Associate degree (A.S.)
Possess active licensure with no disciplinary actions within past 5 years
Active certification in CPR and BLS
Computer skills required with working knowledge of standard office suite software
Detailed understanding of HIPAA and Patient Privacy Act
Bachelor's degree in medical field from four-year College or University is highly preferred
4-6 years clinical experience highly preferred
Knowledge and experience with Electronic Medical Records
Work in industrial business setting
Work Environment: On-site fitness center located in an industrial setting. Steel Toed Safety Shoes required. Hearing Protection may be required depending upon site. Safety Glasses required
