Reporting to the Permanency Supervisor, this individual will have day to day, traditional and home of relative foster care management responsibility.
Completing adoption and guardianship subsidy paperwork within the required timeframes.
Conducting foster home visits, ensuring the child's needs are met, attending staffing's, court and administrative case reviews.
Meeting with the families in their homes at varioushours of the day Mon-Fri as well as be responsible for on call duties as scheduled. Visits and on-call responsibilities may include evenings, nights and some Saturday's, in potentially high risk neighborhoods.
Serve as a conduit of information between all parties to the case and identified stakeholders, including medical, educational and mental health providers.
Develop and implement a plan that meets the needs of the children and the family.
Facilitate placement and delivery of services in collaboration with foster and adoptive families.
Perform case management responsibilities in accordance with state and federal laws, Department of Children and Families Administrative Codes and Operating Procedures, funding source policies and internal policies on child abuse, abandonment, and neglect within required timeframes.
Attend court hearings, prepare and file all court documents within required timeframes.
Must have good driving record and willingness to travel. This very important role requires Case Managers to drive at least 70% of the time.

Bachelor of Social Work or Social Services or related field.
Masters or better in Social Work or related field.
3+ years of FOSTER CARE case management experience
