Provides intensive case management and supportive services to assigned clients who are chronically homeless with a documented disability by assisting them with locating and securing housing for the Critical Time Intervention Representative Payee Program (CTI-RP)
Advocates for clients with regard to financial, state/federal benefits, housing, job training, job referral and/or treatment programs
Completes psycho-social and other assessments in client homes, Agency work locations, or other public locations that are accessible to the clients, including but not limited to; individual interviews, collateral conferences, evaluation of client needs and abilities and following up, when necessary
Maintains up-to-date and accurate client files and databases, as required by program protocols, procedures, Agency policies, licensing and funding bodies
Develops and facilitates scheduled monthly support group meetings, establishes services plans and intervenes, when necessary, using a variety of professional approaches
Knows and complies with the laws and regulations relevant to the position and services
Works collaborative with internal and external partners to obtain services for clients
Participates in community outreach efforts by representing the Agency in community events
Meets with each client, as required by the Program or based on client needs
Assists the Program Director with completing reports and the grant writing process, as needed

Preferred Requirements: MSW. Three years of related experience. LSW or LCSW or LPC or LCPC
Minimum Requirements: Master’s degree in related area. Three plus years of social service experience.
Other Requirements: Driver’s license and reliable transportation. May be asked to occasionally work evenings, weekends or special events, as needed.
