Works independently and consistently exercises discretion and sound judgment in providing individual and group psychosocial counseling Provides assistance to clients who need help accessing and adhering to health care services and who requires a high frequency of contact as well as weekly substance abuse counseling to individuals with complex needs. Provides supportive case management services and/or medical case management services.
Conducts initial intakes and ongoing re-certifications in addition to maintaining appropriate documentation of eligibility for funding bodies.
Develops individual care plans which are clear, concise, and assist clients in meeting their Revises the service plan with the client every six months during the recertification. Ensures timely and accurate submission of care plans as required in all mandatory data systems.
Ensures accurate and timely billing for services provided to client.
Provides services off-site including at the Lake County Health Department and other local facilities; participating in case conferences with treatment coordinators in an effort to develop intensive coordinated
Researches new community resources for unmet needs of clients; tracks and documents referrals, collaborates with other treatment providers and attends all client staffing’s.
Distributes donations of food and clothing to clients. Records the distribution of bus cards. Ensures documentation of all resources provided to clients.
Maintains compliance with Medical Case Management and Department of Rehabilitative Services training requirements.
Organizes and facilitates program events for Participates in community meetings and represents the agency regarding services provided as well as providing accurate and timely reporting to the department director of the meeting discussions.
Preferred Requirements: Master’s degree in Social Service field. Three years of relevant experience. LCSW, LCPC, CADC, Medical Case Management Training.
Minimum Requirements: Bachelor’s degree.
Other Requirements: Position requires a valid driver’s license, automobile insurance. Job duties may need to be performed outdoors in varying weather conditions. May require Spanish Speaking abilities. Completion of Medical Case Management Training within first 6 months after hire. On-going- 12 hours of annual training in Medical Case Management.
