This position will provide overall coordination for care management in the acute care continuum by establishing a comprehensive program which includes social work services, utilization management, and patient case management with discharge planning. Supervises and coordinates the delivery of all Case Management services including acute care services. This includes staff education, hiring, coaching, performance management and mentoring of staff. Responsible for financial management of department including review of productivity and staffing. Responsible for operations of CM department including coordination of UM committee and UM reports. Serves as a clinical resource person for case management services for other hospital departments. Acts as a community liaison with external review agencies, home care providers, durable medical equipment providers, medical staff and other community resources used for discharge planning. Has oversight of Key Performance Indicators for Case Management and provides feedback and direction to Case Management team. Targets are met so that case management remains quality focused.

EDUCATION REQUIREMENTS RN licensed in state of COÂ
Minimum of a BSN required
WORK EXPERIENCE REQUIREMENTS 4 years clinical experience as a Registered Nurse or Social Worker 3 years with progressive experience in care management to include discharge planning and utilization review Prior supervisory/management experience. Coding experience or knowledge preferred.

QUALITY - 20% Provide vision and direction for case management by establishing a defined criteria set for moving patients along the acute care continuum incorporating the disciplines of social work, discharge planning and resource utilization. Demonstrate high competency in care management by providing services that demonstrate exceptional skill in the care management discipline. SERVICE - 20% Assess the case management practices and adapt systems and processes to improve the care of the patient as internal and external expectations change COST - 20% Provide input in contract negotiations regarding the internal and external expectations and roles of care management. Provide liaison reporting with external medical management groups and payors Analyze outcome and variance data in conjunction with Quality Resources and identify potential practice changes, clinical path development opportunities or initiatives to improve the quality and cost effectiveness of care delivered. PEOPLE - 20% Plan, coordinate, and supervise the activities of the Case Management staff by performing managerial human resource functions, developing and meeting a budget, providing a comprehensive orientation plan for new employees and identifying and promoting staff development. GROWTH - 10% Provide resources to enhance the professional practice of case management to include coordination with other health professionals, and staff mentoring. Develop measures of accountability for staff functions to demonstrate the actions and functions of the department. COMMUNITY - 10% Customer Service/Satisfaction â Seeks to provide services that exceed the expectations of customers by; anticipating and responding to customer/associate needs, measuring customer service via surveys, developing action plans to positively impact customer/associate satisfaction and integrating a service focus into daily routines.
