As a case managementÂ at Centura Health, you'll advocate for patients while collaborating closely with interdisciplinary health care partners, facilities and families to ensure the best possible care experience from admission through discharge and beyond by connecting patients to resources that help them maintain and build on positive outcomes.
Utilizes clinical expertise, discretion, and independent judgment in assessing/reassessing, facilitating care coordination,Â utilization management, and patient advocacy. Responsible for assuring medical appropriateness criteria are met for statusÂ and level of care.
What You Bring/Job Requirements
In addition to bringing your whole self to the workplace each day, qualified candidates will need the following:
1 year of RN Case Management experience, required.
Experience working with EMR, preferred.
Working knowledge of regulatory requirementsÂ and accreditation standards, preferred.
Associates Degree in Nursing required; Graduate of an accredited school of Nursing required
RN License required
Physical Requirements -Â Medium Work - (Exert up to 50lbs force occasionally, and/or up to 20lbs frequently, and/or up to 10lbs constantly)
PRN- 6 shifts in 6 weeks required-Â These are 8 hour shifts- 2 weekend shifts required.Â Â The requirements for holidays are 1 winter and 1 summer holiday
