Job Summary: The Care Manager serves as a Bachelor's level registered nurse care manager with responsibility for assessing, coordinating patient care, evaluating and updating the plan of care across the continuum utilizing accepted criteria and guidelines to manage and determine medical necessity of services provided. The RN Care Manager functions as a clinician, case manager, and educator to achieve optimal clinical and quality outcomes by effectively managing care and resources to reduce unnecessary utilization. Collaborates in the development and implementation of performance improvement activities.
Minimum Requirements: BSN Required. Five years experience in one of the following settings: case management, disease management, home health care, hospital or physician office setting.
Ability to work independently, setting priorities to coordinate care plan efficiently. Recognize the role of cultural, social, economic and behavioral factors in accessibility, availability, acceptability, and delivery of public health services, and respond to diverse needs that are a result of these differences. Incorporate strategies for interacting with persons from diverse backgrounds (e.g. cultural, socioeconomic, educational, racial, gender, age, ethnic, affiliation, mental and physical capabilities). Proven leadership skills. Effective communication skills (oral and written). Excellent customer service skills. Effective behavioral and educational strategies, including, but not limited to, motivational interviewing, teach-back method and self management support.
Preferred Qualifications: MSN Preferred.
Bilingual preferred, if defined population requires.
Licensure and Certifications: PA Registered Professional Nurse licensure.
BSN. Case Management Certification within 3 years of hire.
