As a faith-based organization, with over a century of caring for the communities in the Maryland area, Adventist HealthCare has earned a reputation for high-quality, compassionate care. Adventist HealthCare was the first and is the largest healthcare provider in Montgomery County.
Two positions are available within the Care Transitions Management department, Transitional Care RN and Behavioral Health Transitional Care RN.
Adventist HealthCare seeks to hire a Registered Nurse who will embrace our mission to extend God’s care through the ministry of physical, mental and spiritual healing. If you want to make a difference in someone’s life every day, consider a position with a team of professionals who are doing just that, making a difference.
The Transitional Care RN and the Behavioral Health Transitional Care RN collaborate with the patient, physician, case manager, and other healthcare providers to facilitate patient access to healthcare and social services that enhance the patient’s health and well-being and his/her ability to adhere to the prescribed medical treatment regimen in the most efficient and cost effective manner possible.
The Behavioral Health Transitional Care RN works specifically with high and rising-risk behavioral health patients to reduce avoidable utilization, ensure they have access to primary and specialty care, and access to social resources needed to self-manage their health.
The Transitional Care RN provides services to the client in a variety of community and clinical settings. This role works with a multidisciplinary team to identify patients in the hospital at increased risk for readmission and provide care coordination services in the outpatient setting by working with community providers and partners as well as the patient’s family. This role works in conjunction with the Care Transitions and Management Department team to expand access to outpatient services and to support its mission of reducing readmissions by improving transitions of care.
The Transitional Care RN also helps guide and deliver non-medical interventions, community health education, and navigation, in partnership with Community Health Workers and other community partners.
