The Counseling Center Clinical Case Manager is responsible for providing clinical case management services including facilitation of community referrals, coordinating with hospitals following discharge of students, and maintaining a strong referral network in the Syracuse community. The Clinical Case Manager will assist in the development and implementation of processes to increase the likelihood of successful community referrals as well as mechanisms for tracking student utilization of referrals. As part of the Counseling Center’s crisis response team, the Clinical Case Manager conducts initial assessments, provides triage services, assists in the management of high acuity and/or crisis situations and maintains a small clinical caseload. The Counseling Center embraces social justice as a core value, and the Clinical Case Manager must demonstrate high levels of cultural competency. The Counseling Center operates within an integrated system, and the Clinical Case Manager will have the opportunity to work closely with our Health and Wellness partners in the Health Center, Health Promotion, Recreation Services, and Student Assistance.
