As a Case Manager in Float Surgical Services, you will utilize your advanced practice nursing skills to assess, educate and coordinate patient care. You will collaborate and consult with a multi-disciplinary clinical team as well as patients and families to ensure safe and effective coordination of care. You will be responsible for developing and implementing individualized care plans using evidence-based risk-stratification tools to ensure delivery of safe, high quality, efficient, and cost-effective care. This role involves performing utilization review functions while assuring the delivery of exceptional care from admission, to discharge and post-care. You will also participate in performance improvement projects/processes and identify and implement a comprehensive DC plan for high-risk patients.
QualificationsWe’re seeking a self-directed nursing professional with a BSN, California RN license and: At least three years of acute hospital experience, including case management, utilization management, or discharge planning. Team leading or management experience. Understanding of the functions and operations of large university teaching hospital. Exceptional communication, interpersonal, diplomacy, organizational, and analytical skills. Skill in abstracting and interpreting medical information from patient records. Working knowledge of laws, rules, and regulations regarding utilization review and discharge planning functions, including Medicare, Medi-Cal, and CCS. Ability to develop an individualized case management plan that addresses physical, vocational, psychosocial, financial, and educational needs. Computer proficiency. Creativity in problem solving, systems planning and patient care management. Ability to thrive in a high-volume work environment.
