OBJECTIVE/OVERVIEW Case Management is defined in State Statute as providing coordination and access to services required by people experiencing serious and persistent mental illness (SPMI, adults) or serious emotional disturbance (SED children). The clinic has a contract with Hennepin County and various Managed Care Organizations to provide these services to eligible patients who reside in Hennepin County. The scope of the work and the requirements for supervision of the work is found in MN. Statute and in the MN. Health Care Provider Manual published by the Dept. of Human Services.
REPORTING RELATIONSHIPS: This position reports to a Behavioral Health Program Manager
ESSENTIAL FUNCTIONS & PERCENTAGE FOR EACH FUNCTION:
1. Provide adult or child case management services 100% Assessment: 1. Gather information to identify the recipient’s functioning level and need for services in all domains. 2. Complete and regularly review the written functional assessment 3. Routinely contact and monitoring or communicate with the recipient, family members, providers, or other relevant persons about the status of the recipient, the individual service plan or progress in achieving the goals of the service plan. 4. Maintain records related to the recipient’s status and functioning
Expected Outcomes: • Accurate understanding of client’s mental illness, strengths and needs. • Functional Assessment completed with description of current status, description of functioning and link between the functional impairment and mental illness. • Client readiness to utilize services will be assessed.
Planning: 1. Develop a service plan (Individual Community Support Plan) with the client (and family) designed to meet the needs of the client which were identified in the assessment. 2. Develop and revise goals as needed including written service agreements and routine case supervisory activities 3. Receive clinical supervision for identified recipients with SPMI or SED
Expected Outcomes: • Tailor planned services to individual needs and preferences. • Full range of resources will be utilized including natural and professional supports. • Clients will be discharged when they have met their goals and have sufficient supports in place.
Referral, Coordination, Monitoring: 1. Make effective linkages with natural and social supports as outlined in the ICSP. 2. Monitor client use of services to assure that services are meeting client needs as planned.
Expected Outcomes: • Facilitate effective referrals providing support to the client as needed; including arranging and/or providing transportation to clients when appropriate. • Client will have needed financial resources to obtain and utilize the services. • Ongoing contact between CM and treatment team to monitor client progress on goals. • Adequate oversight of client services to avoid duplication and gaps in service.
Working Relationship with Client: 1. Develop relationship and trust in order to support the client in achieving their goals.
Expected Outcomes: • Demonstrate ability to engage client and develop trust. • Understand and use the recovery model and motivational interviewing • Manage role clarification, balancing CM and client roles and responsibilities. • Demonstrate skills and strategies in working with people with mental illness.

MINIMUM/ESSENTIAL QUALIFICATIONS: 1. Training/Experience (Must meet qualifications in one of the following ways): A. BA/BS in one of the behavioral sciences or related fields including but not limited to social work, psychology, or nursing from an accredited college or university with at least 2 years of experience or a combination of related education and work experience to equal six years; AND: At least 2,000 hours of supervised experience in the delivery of services to persons with mental illness OR is fluent in the non-English language group of the ethnic groups CUHCC serves. B. has at least 6,000 hours of supervised experience in the delivery of services to persons with mental illness; C. is a graduate student in one of the behavioral sciences or related fields and is formally assigned by an accredited college or university to an agency or facility for clinical training or D. holds a master’s or other graduate degree in one of the behavioral sciences or related fields from an accredited college or university and has less than 4,000 hours post-master’s experience in the treatment of mental illness.
A mental health practitioner for a child recipient must have training working with children. A mental health practitioner for an adult recipient must have training working with adults.
2. A valid Minnesota driver’s license and access to a vehicle with insurance coverage to make home and agency visits and transport clients as needed. 3. Strong time management and organizational skills. 4. Customer service knowledge and skills. 5. Ability to de-escalate mental health crisis situations. 6. Capacity to effectively communicate in difficult clinical situations and to motivate patients and others. 7. Proven ability to establish and maintain positive rapport with a diverse variety of clients and professionals. 8. Ability to set appropriate boundaries and limits for best practice. 9. Ability and willingness to provide services to children, adolescents, adults. 10. Ability to utilize and complete all documentation processes in an electronic billing and health record format in a timely manner. 11. Excellent English written and oral communication skills. 12. Skill and commitment to customer service. 13. Ability to work both independently and as part of a multidisciplinary team. 14. Respect for and ability to work with a diversity of patients and staff.
PREFERRED QUALIFICATIONS: 1. Experience working with clients with mental illness. Experience working in community mental health clinic. 2. Proficient in one of CUHCC’s dominant languages: Vietnamese, Lao, Spanish, Hmong or Somali.
