Rhode Island School of Design is actively seeking a Nurse Case Manager to join Health Services. RISD's Health Services provides high-quality, confidential healthcare services to students in an ambulatory care setting, including RN triage for first aid/emergent care, acute care, and referrals to specialists. Our qualified and knowledgeable providers give personal attention specifically to all healthcare needs.
Reporting to the Health Services Administrative Director, the Nurse Case Manager maintains high student contact and manage an ongoing caseload of students with complex medical concerns that may affect their experience at RISD. This position performs nursing care and individualized health education based on assessment of the student's needs. Responsibilities will include regular contact with high-need students, telephone and walk-in triage, referral coordination with outside medical providers, and engaging with other departments including RISD staff, faculty, and administration, to optimize the student experience.
Meet 1:1 with students to triage presenting problem. May refer to NP as needed.
Responsible for the accurate and timely documentation of medical records as appropriate. Run reports, standard and ad-hoc, from RISD's EMR program to manage cases and update statuses. Utilizing the college's EMR system, monitor, track, record, and generate reports on student immunizations. Ensure compliance with regulations/legislation.
Coordinate transportation in emergent situations.
Responsible for reviewing on-call reports and for coordinating follow-up care as appropriate.
Consult and advise students on self-care management plans.
Receive and review incoming lab results, x-rays and other medical reports and coordinate follow up plans.
With an understanding of presenting issues on campus and of RISD's culture, provide individual health counseling and education on various topics, including, but not limited to nutrition, eating concerns, alcohol and drug awareness, contraception, STD's, HIV and pretests, and wellness initiatives.
Ensure Health Services educational materials, brochures, etc. are current and reflective/responsive to community needs.
Arrange referrals for students needing outside resources from other areas, both inside and outside the college community.
Facilitate medical leave and return of students. This includes collection and evaluation of documentation, contact with off-campus treatment providers, interviews with students who wish to return, and making recommendations. Provide follow up and track students returning from medical leave of absence to ensure student compliance with recommendations and adequate on- and off-campus support.
Attend conferences and workshops, as approved, to stay current with best practices.
Perform other related duties as may be required.
Registered Nurse professional license, current in state of Rhode Island
Minimum five years previous nursing experience required
Experience in adolescent, young adult and/or college health or ambulatory care setting preferred
Strong communication and organizational skills, with a background in direct patient care and education
Experience with and commitment to working with diverse clients including international students, individuals from ethnically/racially diverse backgrounds, and individuals representing various sexual orientations and gender identities
Experience managing response to crisis situations
Strong attention to detail, excellent communication skills, ability to quickly synthesize information from a variety of sources in order to make informed recommendations for the best welfare of the students
Ability to handle and address sensitive, personal situations with confidentiality, decorum, and protocol.
Valued but not required skills and experience:
Knowledge of current case management trends and literature preferred
