Responsible for the coordination, supervision, and administrative oversight of the care management team. Functions as an expert clinical practitioner, resource, advisor and leader for the members of the case management team. Supervises and monitors professional and support staff and ensures that effective care coordination and case management practices are consistent with Medical Center policies, and applicable regulations and guidelines. Monitors quality of documentation and assessments with the patient/family by regular auditing. This role will allow your creativity to drive change and inspire the staff. The ideal candidate will enhance outcomes through staff coaching and development.

BSN required; MSN Preferred
5 year hospital Case Management experience required, 1 or more years leadership experience desired
RN License is a must; ACM, or CCM Certification is a plus
