Position Summary: This position supports the mission of Wesley Medical Center in optimizing the quality of life of all those served by expanding beyond the traditional focus on medical needs to the needs of the whole person. The Social Worker Case Manager is responsible for assessing the psychological, social, cultural, and/or financial needs that impact the high risk patient's health and recovery. Has the ability to utilize social services, counseling and clinical knowledge in the assessment of these needs. Responsible and accountable for coordination of services through an interdisciplinary process which provides a clinical and psychosocial approach through the continuum of care and balances individually identified patients and family needs with the efficacious and cost effective utilization of resources. In collaboration with the RN Case Manager, patients will be assessed to determine appropriateness of admission, continued hospitalization, as well as appropriate level of care. Discharge planning will begin at the time of (or prior to) admission, and reassessed throughout the course of hospitalization. Knowledge of payer source implications on discharge plan, resources and placement options to maximize patient’s successful dismissal.
Responsible for assessing the psychological, social, cultural and or financial needs that impact patients at high risk for successful transitions on dismissal
Assesses relevant clinical and age related information to assist in the development and implementation of timely treatment and discharge plans
Tracks and trends barriers to care; makes recommendations and develops action plans to improve processes and systems
Maintains rapport and effective communication with the healthcare team and patient/family so the plan of care can be discussed objectively, problems identified, recommendations and adjustments made to the plan as needed
Fosters the patient/family's decision making and autonomy and act as a patient/family advocate
Intervenes to resolve system problems that influence billing
Acts a a liaison through effective and professional communications between and with physicians, patient/family, hospital staff and outside agencies
Applies knowledge of state and federal laws, professional practice standards and other regulatory guidelines
Assesses cost-effective options which maximize patient outcomes and meet patient/family needs
Required Registration, Licensure or Certification
Current licensure as a social worker by the Kansas Behavioral Sciences Regulatory Board as a LBSW
Current Basic Life Support (BLS) verification course, as specified in policy
Required Education or Experience
Bachelor's degree in social work from an accredited institution
Knowledge about internal, community and post-acute care resources
Preferred Education or Experience
Certification in social work or case management preferred
Master’s degree in social work from accredited institution
Recent hospital social work or case management experience
