Within the inpatient setting to assess and address patient and family needs related to effective and efficient movement through the hospital system and beyond. Perform initial assessment on all patients and begin discharge process from day one of patientâ™s stay. Complete level of care assessment to ensure patients are receiving the appropriate level of care, and to ensure that the patient transitions through their stay without barriers or delays in care delivery. Maintain expert knowledge of InterqualÂ® Level of Care criteria.Work closely with the Admitting Physicians to ensure appropriate documentation of clinical information necessary to support or determine alternative level of care. Coordinate discharges home and facilitate transfers to external placement facilities.
