We are seeking a Case Management Assistant to support the Inova Health System Case Management Department.
The job duties for this position include:
Communicates and documents accurate clinical and financial information in both verbal and written forms in order to assist Case Manager
Interprets and communicates payor requirements to Case manager.
Receives and documents benefit and contract level requirements in order to assure appropriate utilization of benefits and services.
Supports the case management outcome model through clinical, clerical and support services in order to assist with the planning, assessment and implementation of appropriate patient transitions.
Accurately arranges services for assigned patient population including Home Health, Durable Medical Equipment, ambulance, housing, skilled nursing facility and others in order to assure appropriate continuing care services are in place for patient transitions.
