The Case Manager provides direct individual and family casework services with an emphasis on meeting the psycho-social needs of clients. This includes the assessment of service needs, service planning and service delivery for assigned clients within UCAN policies, procedures and practices.
Assessment and Service Planning • Completes all required assessments and reassessments of assigned clients within the guidelines and timeframes established by UCAN. • Develops a written service plan within required timeframes for each assigned case that adequately and appropriately addresses the client/family’s unique service needs. • Implements client service plans, monitors service plan progress, and reviews progress with supervisor and DCFS as required. • Demonstrates insight regarding client strengths and needs and incorporates this into service planning and assessment.
Documentation and Reporting • Makes frequent reports to supervisor on progress of client services; maintains statistical records and performs other record keeping functions as required by UCAN. • Maintains accurate, complete and timely client records as required by UCAN policy and procedures. • Reports client progress in court proceedings through clear written reports submitted as required, and through accurate verbal testimony. • Enters, updates and maintains accurate data in the DCFS SACWIS and Profile databases, as required. • Reports all possible critical incidents and unusual incidents to supervisor, management, or DCFS as required by UCAN or by mandated reporting law.
Organization and Service Delivery • Provides linkage to other appropriate resources within specified referral procedures; secures all written agreements in accordance with guidelines established by contracting agencies and other outside sources. • Obtains and evaluates information necessary to implement client services. • Recommends and obtains supervisor approval of limited financial assistance for client needs. • Creates organized plans so that all casework tasks can be completed as required. • Responds in a timely manner to all written correspondence, email and phone call inquiries regarding clients.
Building Relationships and Networks • Maintains client face-to-face contact through required number of home and office visits. • Engages resistant and/or mandated clients in services. • Provides outreach to client, family members and other significant people in the client’s life to maintain contact and to develop resources for the client. • Develops effective helping relationships with clients and their families. • Maintains required contact with relevant family members, assesses their need for services, and works on relationship building between client and family. • Plans, coordinates and supervises required family or sibling visits. • Contacts courts, DCFS, and other service providers or community organizations to coordinate client services or to make referrals.
Job Knowledge Demonstration • Knowledge of DCFS, UCAN, Medicaid, COA, and legal requirements for service delivery and documentation. • Studies, learns and uses UCAN and DCFS procedures for providing services to clients. Explains UCAN’s services to clients and families and assists them in the utilization of all available resources. • Assesses and monitors the safety of clients in current living arrangement and makes any needed recommendations and referrals for appropriate placement. • Assesses permanency options; recommends permanency plan for clients; and implements plan to achieve permanency for clients. • Develops a working knowledge of community resources to use in serving clients and maintains productive working relationship with these resources.
Leadership • Provides mentoring/shadowing to new case managers and assists supervisor in the training of new case managers. • Creates and/or maintains resource listing for program (DCFS, UCAN, referrals, etc). • Represents program at new employee orientation and other agency functions • Covers for other workers when needed
Education /Job Experience/Certification: • BSW or BS/BA in Human Service field with 3-5 years similar casework experience • BSW or BS/BA in Human Service field with 2+ years child welfare casework experience • MSW or MS/MA in Human Service field with 0-2 years casework experience. • Licensed Child Welfare Services Employee or license eligible.
Special Knowledge and Qualifications: • Ability to provide leadership and guidance to other staff members. • Is open and respectful of cultural and socioeconomic characteristics of clients and is willing and competent to work with a diverse client population. • Effective verbal and written communication skills. • Ability to work both independently and a part of a high functioning team. • Experience or ability to use computers for data entry, communication and report writing • Car and valid Illinois driver’s license required.
