Position Description: The Health Care Case Manager works within HOW’s Permanent Supportive Housing program to assist clients in navigating the health care system. Participants in HOW’s programs are formerly homeless with a physical or behavioral health disability. This position focuses on case management with a head of household who has a Serious Mental Illness and needs assistance in accessing primary care and behavioral health resources.
Duties and Responsibilities
One on One Case Management Services
Provide case management to a case load of HOW clients. Clients have histories of homelessness and physical and behavioral health disabilities.
Assess, identify and respond to emergency or crisis needs of program participants
Screens for and assists with accessing financial resources such as income, SNAP, etc.
Work collaboratively with internal HOW teams, including employment, youth services and housing
Implement client engagement strategies to strengthen the case management relationship
Complete individualized work plans with clients
Maintain required documentation in relevant databases
Support an individualized approach to skill development and career advancement
Health Care Coordination
Assist clients with accessing health care appointments, including scheduling, reminding, coordinating transportation and escorting
Assist clients with enrollment into and use of the patient portal, including review of chart summary and lab results, communication with providers, and appointment scheduling.
Discuss, confirm, and assist as needed with MCO assignment of primary care site and provider
Ensure that participants obtain prescribed medication, equipment, and supplies, by assessing and assisting with removing barriers
Complete all relevant data collection and reporting tools
Document activities to meet Rule 132 billing standards
Maintain required professional knowledge and job skills
Participate in required staff meetings and training
Maintain regular and predictable attendance during standard business hours, with willingness to work flexible hours as required by the position’s responsibilities
Perform other duties as assigned or approved by direct supervisor
POSITION REQUIREMENTS AND QUALIFICATIONS
Bachelor’s Degree preferred or commensurate experience in the employment and training field
Must have relevant experience in case management and/or health care coordination
Knowledge of barriers for persons with disabilities in accessing the health care system
ESSENTIAL KNOWLEDGE AND SKILLS
Medicaid billing experience a plus
Excellent communications skills, organizational and research abilities
Comfortable working within a collaborative team setting
Proficient in Microsoft Office programs.
Must possess valid driver’s license, reliable vehicle and proof of automobile insurance
Must pass applicable background tests
