BA or BS degree in Psychology or Sociology is required. The case manager interacts with a variety of treatment professionals to coordinate services as needed. The case manager works to link consumers with community agencies in order to ensure client’s medical/health, educational, vocational, and housing needs, as well as securing and maintaining benefits. This individual also facilitates other skills training services necessary for successful independent living, such as obtaining and attending medical appointments, assistance with activities of daily living, and arranging transportation services. If a client is on psychotropic medication, the case manager works to ensure that the client is obtaining and taking the medication as prescribed. They may also coordinate treatment with family members and significant others. The case manager is responsible for completing documentation and assessments as needed. Will also be responsible for maintaining payee responsibilities for persons. The individual must attend all daily ACT team meetings, and will work on the rotating on call weekend/holiday schedule.
