Position Summary: The Case Manager is responsible and accountable for coordination of services through an interdisciplinary process, which provides a clinical, financial, and psycho-social approach through the continuum of care. Balances individually identified patient and family needs with the efficacious and cost effective utilization of resources. Through concurrent case management, patients will be assessed to determine appropriateness of admission, continued hospitalization, clinical resource consumption, progressive care management, as well as appropriate level of care. Discharge planning will begin at the time of (or prior to) admission, and reassessed ongoing throughout the course of hospitalization. The Case Manager performs delegated duties consistent with the scope of practice and functions within the philosophy and overall plan of care at RMC. The Case Manager displays positive support for the values and mission of RMC.
Licensure/Certification/Registration: Current licensure in the State of Colorado as a Registered Nurse, or current active multi-state Registered Nurse license
Education: Graduate of an accredited School of Nursing.
Experience: 3-5 years recent clinical or case management experience.
Knowledge, Skills and Abilities: Must have the ability to assess information, apply critical thinking processes to determine course of action, and relay this information to the interdisciplinary team. Requires excellent human relations and oral/written communication skills. Must be able to prioritize workload, and develop efficient methods of managing time.
Degree of Supervision Required: Minimal; expected to be self-directed.
Orientation Period: Minimum of three weeks training/orientation contingent upon experience. Extensive orientation may be required and will be determined by Department Director / Designee.
