South Side Help Center’s mission to serve people and strengthen communities is done by providing direct prevention and intervention services designed to enhance the quality of life for individuals, families and community. With a compassion for people, commitment to service, motivation to affect change, cultural awareness and knowledge about your role and responsibilities as a Medical Case Manager (MCM) you will help to fulfill this mission by providing a full range of services that are client-centered and focused on improving health outcomes.
The Medical Case Manager (MCM) will provide of a range of client-centered services focused on improving health outcomes in support of the HIV care continuum. Using this client centered focus, the MCM will promote and support client independence and self-sufficiency through services that includes treatment adherence counseling, coordination and follow-up of medical treatments, client advocacy and referrals to needed services. Your goals as the MCM is to ensure that clients with HIV/AIDS have timely access to comprehensive medical care and essential supportive services; prevent disease transmission and delay the progression of HIV.
The MCM may also contribute to the success of South Side Help Center by lending a hand when needed, showing flexibility, paying attention to details, adapting to changes when needed and thinking of their role as a part of the larger SSHC team rather than exclusive.

Founded in June 1987, South Side Help Center (SSHC) is purposed to help people of all ages embrace a lifestyle of prevention against mental, physical and social ills by providing positive, healthy alternatives so that community residents can lead productive lives. AIDS Healthcare Foundation
The Los Angeles-based AIDS Healthcare Foundation (AHF) is a global nonprofit organization providing cutting-edge medicine and advocacy to over 1,000,000 people in 43 countries. We are currently the largest provider of HIV/AIDS medical care in the U.S.
AHF funds its mission to rid the world of AIDS through a network of pharmacies, thrift stores, healthcare contracts and other strategic partnerships. Generating new, innovative ways of treatment, prevention and advocacy has been the hallmark of our success. We are currently leading a mass testing initiative to identify and treat the 25 million people who are unaware they are infected.
By advocating big goals–such as having 1 billion people tested each year–AHF hopes to eliminate older, more time-consuming methods.
