Serve as a Patient Advocate at WakeMed as a Manager, Case Management
As the Manager, Case Management, you will oversee the day to day operations of the point of entry for the Patient Case Management department and work collaboratively with departmental leadership and the physician advisor to carry out department goals. Responsibilities include:
Implementing and maintaining daily departmental operations with consistent standards of practice.
Maintaining clinical competency and serving as clinical resource to staff of the department, physicians, and nursing leadership to support patient progression and appropriate resource utilization throughout the system.
Collaborating with physicians, nursing, members of the interdisciplinary team, Patient Financial Services, Patient Access and other departments to ensure that quality services are being provided timely and to serve as a resource to resolving barriers.
Analyzing data to improve the effectiveness of the Patient Case Management program.
