Position Summary: The Director of Care Support/Management will collaborate with the VP of Population Health to provide leadership and oversite for a suite of developing programs at HealthPoint – The Care Support Programs. Programs include high quality care transitions from emergency rooms and hospitals, care coordination support, and RN case management functions. The Director of Care Support provides leadership and oversite for a professionally diverse group of staff including social workers, RN case managers, and patient engagement coordinators. Position will also build and manage programs using a health equity lens that specifically addresses care gaps with an understanding of the role social determinants of health play in health outcomes.

Your contribution to the team includes: • Collaborate with VP of Population Health to develop staffing model, workflows, and a monitoring and evaluation system that improves the health of vulnerable and high-risk patients. • Provide intensive care support to address social determinants of health with the goal of improving quality of care for high risk and vulnerable patients by proactively closing care gaps. • Provide day to day management, coaching, development and performance feedback for care transitions, care coordination, and case management staff. • Continuously improve program workflows using LEAN and IHI model for improvement methodologies. • Work with regional and clinic based business and clinical leaders to maximize patient and care team satisfaction with all programs. • Work closely with VP of population health to understand and support organizational care support priorities including quality and financial goals driven by external stakeholders. • Work with external partners like Managed Care Organizations, the state, the department of health, and local hospitals and community-based organizations in the best interest of HealthPoint Patients. • Work with business analytics and IS team to analyze and understand success, build care support dashboards and monitor/evaluate program.

Must have’s you’ll need to be successful: • Minimum education desired is a Bachelor’s Degree in Nursing, Public Health, or equivalent combination of education and experience. • Minimum five (5) years of healthcare experience including direct patient contact with at least three (3) of those years being in a healthcare management role. • Intermediate level of Microsoft Word, Excel and Outlook required. • Proof of immunity to varicella, measles, mumps and rubella is a condition of employment. Prospective employees must provide proof at time of offer.
If you know about the following it’s a plus: • Master’s Degree in Nursing or an Advanced Practitioner Degree (ARNP, PA-C) is highly preferred.
