(Hospital Case Management Experience Preferred)
As a Nurse Case Manager, you are responsible for providing patient case management services to assigned populations. Responsibilities include:
Utilizing a collaborative interdisciplinary approach to anticipate, integrate, and coordinate the patient’s plan of care, discharge plan, and re-entry into the community.
Meeting the needs of patients in order to optimize clinical and resource outcomes and monitoring the effectiveness of the plan and its outcomes.
Identifying at-risk patient populations who require patient case management services, in collaboration with the social worker.
Serving as a patient/family advocate to act as a communication link between the physician, patient, family, health care team, community resources, and payers to reduce redundancy and fragmentation of care.
Providing bio-psychosocial assessments to identify patient needs and assisting in the development of the plan of care.
Anticipating potential delays in the health care process and acting proactively to avoid these delays.
Ensuring that necessary services are provided at the most appropriate level of care and that there is a smooth progression of the patient throughout the health care system and into the community.

Graduate in Nursing Required
