(Hospital Case Management Experience Preferred)

As a Nurse Case Manager, you are responsible for providing patient case management services to assigned populations. Responsibilities include:

• Utilizing a collaborative interdisciplinary approach to anticipate, integrate, and coordinate the patient’s plan of care, discharge plan, and re-entry into the community.
• Meeting the needs of patients in order to optimize clinical and resource outcomes and monitoring the effectiveness of the plan and its outcomes.
• Identifying at-risk patient populations who require patient case management services, in collaboration with the social worker.
• Serving as a patient/family advocate to act as a communication link between the physician, patient, family, health care team, community resources, and payers to reduce redundancy and fragmentation of care.
• Providing bio-psychosocial assessments to identify patient needs and assisting in the development of the plan of care.
• Anticipating potential delays in the health care process and acting proactively to avoid these delays.
• Ensuring that necessary services are provided at the most appropriate level of care and that there is a smooth progression of the patient throughout the health care system and into the community.

Work Schedule:

• Monday – Friday, 8:00am – 4:30pm
• Weekends as needed
• Call requirements: as needed

Education Requirements:

• Graduate in Nursing Required
• Bachelor’s Degree Required
• Bachelor’s Degree in Nursing Preferred

Experience Requirements:

• 3 Years of Nursing in a Related Area Required
• 5 Years of Nursing in a Related Area Preferred
• Direct Position Experience in Clinical – Case Management, Discharge Planning, Accounting, Finance, Third Party Review, or Home/Community Health Preferred

Licensure/Certification Requirements:

• RN License Required
• Certified Case Manager or Commission for Case Management Certification Within 1 Year of Hire Required
