Reporting to the Director of Case Management, the Case Manager is a licensed RN and is responsible for the coordination of the continuum of patient care in accordance with the vision and goals of Doctors Medical Center. The Case Manager ensures quality, cost-effective patient care through planning; communication, critical thinking and facilitation of the multi-disciplinary patient care team.

  • CA RN license

  • 2+ years Case Management experience in an Adult Acute Care Hospital
