Essential Job Functions 

1. Provides basic health and medical assessment and education.
2. Assists with coordination of health care provided to consumers in the community.
3. Performs psychiatric and medical assessment, treatment, and education.
4. Provides 24-hour on-call phone support for HF consumers on a rotational basis.
5. Performs treatments and laboratory procedures independently and/or as requested by the health care provider.
6. Rigidly adheres to Universal Precautions per CDC guidelines and OSHA regulations.
7. Rigidly adheres to HIPAA regulations per standards developed by the Department of Health and Human Services.
8. Provides appropriate patient education that is tailored to both the individual’s learning style, education, and/or reading level, and is sensitive to their health beliefs.
9. Assesses and monitors consumers’ ongoing psychiatric, medical, and social needs.
10. Oversees medication monitoring and medication administration under the supervision of the designated provider.
11. Collaborates with internal and external providers to coordinate psychiatric treatment with medical treatment; acts as a psychosocial educational resource by assisting consumers in learning about their mental illness, physical health conditions, medications, and how to protect their overall health.
12. Manages pharmaceutical and medical supplies.
13. Makes home visits and delivers medications.
14. Administers intramuscular medications.
15. Monitors client’s vital signs.
16. Coordinates providers’ orders and medication refills.
17. Maintains accurate and timely clinical records consistent with CCH standards.
18. Participates in team meetings, in-services, and supervisory sessions as required.
19. Must demonstrate competency in agency databases including HMIS and EHR.
20. Record all clinical services in a timely and complete fashion in the EHR
21. Maintain up to date records in EHR
22. Record telephone and other clinical communications as needed in the EHR
23. Manage tasks, PAQ, and scanning as needed to have a complete and current chart
24. Maintain accurate information regarding client’s insurance status and complete CCARs/DACODs and other clinical authorization documentation as needed
25. Utilize the EHR and any future upgrades as directed
26. Must be accessible by cell phone during business hours.

?Qualifications Summary 

1. Graduation from an accredited school of nursing.
2. Licensed as a BSN or RN in the State of Colorado.
3. Certification in Basic Life Support/CPR.
