Case Management Coordinator - CA
Job DescriptionThis is a full-time telework role, however, will require frequent field-based travel in San Diego, CA once COVID restrictions are lifted. May require office-based work once COVID restrictions are lifted. The Case Management Coordinator utilizes critical thinking and judgment to collaborate and inform the case management process, in order to facilitate appropriate healthcare outcomes for members by providing care coordination, support and education for members through the use of care management tools and resources. Required core business hours for this position are Monday through Friday, 8am to 5pm PST.Evaluation of Members: - Through the use of care management tools and information/data review, conducts comprehensive evaluation
